A. Freshmen

1. Pass the FEATI
   College Admission Test (FCAT)
2. High School Card (Form 138)
3. Certificate of Good Moral Character
4. 3 pcs. 2X2 pictures with name tag
5. Birth Certificate(photocopy)
6. Testing Fee of Php 300.00

B. Transferees

1. Pass the FEATI College Admission Test (FCAT)
2. Transfer Credentials / Honorable Dismissal
3. Certificate of Good Moral Character
4. Original Transcript of Records
5. 3 pcs. 2X2 pictures with name tag
6. Birth Certificate (photocopy)
7. Testing Fee of Php 300.00

C. Foreign Students
ADMISSION REQUIREMENTS FOR STUDENTS FROM ABROAD

1. Application letter (2 copies)
2. Must be 18 yrs of age and above
3. School credentials (Original and three photocopies)
4. Birth Certificate and or passport pages where name, photo, birthdate, birthplace appear.
5. Character certificate
6. Affidavit of financial support with Bank Statement
7. Personal History Statement (PHS) five copies
8. 2 pcs 2”x2” latest picture with name tag
9. Pass the FEATI College Admissions Test (FCAT)
10. Testing Fee of Php 300.00

* The FEATI College Admission Test (FCAT) result is released on the same day.
